An enhanced Form to Contacts. On every form submission it creates a real Google
Contact — with an extended field set and deduplication, so re-submissions enrich
one record instead of spawning duplicates. Leads stop being stranded in a
response spreadsheet the moment they arrive.

WHAT IT DOES
• Create or update on submit: an on-form-submit trigger fires for every new
  response and creates (or updates) one Google Contact automatically.
• Extended field set: map any question to givenName, familyName, email, phone,
  organization, jobTitle, address, birthday, or notes.
• Dedup / update by email: toggle "update existing contact when email matches" —
  a matching submission patches that contact instead of creating a duplicate,
  searching both saved and "other" contacts.
• Group / label: optionally add every synced contact to a named group, created
  automatically if it doesn't exist.
• Flexible birthday parsing: accepts YYYY-MM-DD, YYYY/MM/DD, and MM/DD/YYYY;
  anything else is preserved as text rather than dropped.
• Manual test: "Sync latest response (test)" pushes your most recent submission
  so you can confirm the mapping before going live.
